Eye Care for the Adirondacks maintains custodial responsibility for certain medical records originating from North Country Eye Associates (Dr. Beauvais).
Requests for copies of records are processed in compliance with HIPAA, New York State Public Health Law §18, and all applicable regulations.
To obtain medical records, glasses prescriptions, or contact lens prescriptions, a written authorization is required. The Medical Records Release Authorization form is available at https://www.nycourts.gov/forms/hipaa_fillable.pdf. If you are unable to access the form online, you may email ncea@eyecareadk.com to request that the form be sent to you by email or by mail after you provide your preferred mailing address.
Patients are also welcome to schedule an appointment with Eye Care for the Adirondacks at one of our three locations in Malone, Saranac Lake, or Plattsburgh, NY.
Notice Regarding Workers’ Compensation Coverage
Effective January 1, 2026, Eye Care for the Adirondacks will no longer be accepting new Workers’ Compensation Benefit (WCB) cases.
We will continue to provide care for existing Workers’ Compensation patients already established with our practice.

As we adjust our schedules and office flow to support recommendations by the CDC and the Department of Health, we ask for your patience and understanding as we move through different phases of reopening.
Here’s what you can expect during this time:
- 1. Patients and staff will be required to wear masks (cloth or surgical) while in the office during office operational hours.
- 2. If you need assistance during your visit, we ask that the person who brings you in waits in his/her car until your appointment is over to help us limit the number of people in the clinic at any one time.
- 3. If you have a caregiver who needs to attend the visit with you, he or she will also need to wear a mask. Please inform staff if this is the case.
- 4. We will gather information through as many contactless ways as possible and will establish handwashing protocols for everyone.
- 5. We will communicate any additional changes as we move through this process and will continue to share information with you.

In response to the state of emergency declared at the federal level yesterday evening and CDC requirements for preventing the spread of coronavirus all Eye Care for the Adirondacks clinic doctors, as of March 18 to May 31, and will be postponing eye examination visits and procedures that can be safely delayed, particularly in elderly patients and those with non-urgent eye care problems. All non-urgent scheduled patients will be notified of the change. We are significantly reducing our patient schedules to provide critical eye care visits with established and emergency patients only.
All elective surgical procedures performed for patients scheduled in the Cataract Center for the Adirondacks, scheduled from March 18 to May 31, will be rescheduled for a later date.
Our optical and contact lens centers will be closed from March 18 to May 31. Eyeglasses and contact lenses that are completed and ready to be dispensed can be mailed out if patients call the location they received their services in.
